Both members and non-members can now participate!  Your participation is important and will improve the usefulness of our reports for you and your practice. 

Our goal is to conduct the most extensive surveys available to dental professionals.  Each submission is immediately added to the database. Although we have no way to ensure that the information submitted is absolutely valid, we do monitor all submissions and compare them to others within the same categories.

Reminder Revised I-9 forms

• Effective 4-3-2009 employers must use the revised I-9 form.
• A blank Form I-9 may be reproduced, provided both sides are copied. The Instructions must be available to all employees. Employers must retain completed Forms I-9 for three (3) years after the date of hire or one (1) year after the date employment ends, whichever is later.
• The revised I-9 form may be obtained by clicking the USCIS.gov link posted on our links page.
